Want to stay connected but don’t know how? Our Social Support is designed to help you stay engaged with the people, places, and activities that matter most.  

 

Our services include (but are not limited to): 

 

  • Transport out and about, including to and from appointments
  • Support with shopping, errands, and outings
  • Visiting your local shops and/or community centres
  • Companionship
  • Support to participate in hobbies, interests, and group activities
  • Encouragement to maintain social connections

Do you provide transport to appointments and activities?

Yes, we can assist with transport to medical appointments, social activities, shopping trips, and community events, ensuring you can stay active and engaged.

Who can access social and community support services?

These services are available to anyone who would benefit from assistance to stay connected and engaged, including older adults, people living with disability, and those needing extra support to get out and about.

Can I choose the activities I want to participate in?

Yes, you’re always in control. We support you to take part in activities you enjoy, whether that’s social outings, hobbies, or community programs.

Can support workers accompany me to appointments or events?

Yes, our support workers can accompany you to appointments, social events, or outings, providing assistance and reassurance along the way.

What if I feel nervous about going out or meeting new people?

That’s completely okay. We take a gentle, supportive approach, starting at your pace and gradually building your confidence in social and community settings.

Need a New Care Provider? Here’s How to Switch

If your current care no longer feels right, changing providers can help you access the support, reliability, and service you deserve. 

1. Find a New Provider

Start by researching and choosing a provider that better suits your needs, preferences, and level of support with availability to take you on.

2. Notify Your Current Provider

Let your existing provider know you intend to change and review any required notice periods or exit steps (usually 2 weeks).

3. Transfer Your Care

Contact My Aged Care to request your referral code for changing providers, then give this code to your new provider when setting up your new agreement.
